Transportation Service Agreement

The terms below define service responsibilities, passenger requirements, billing, safety, and legal obligations.

Service and Compliance

What TJ Legacy provides and what the Client must provide before service begins.

Scope of service

This agreement creates a prepaid or invoice-backed transportation service bank for business transportation, including employee, client, patient, senior, daycare, school-adjacent, airport, appointment, errand, and special-trip transportation as scheduled and accepted by TJ Legacy LLC Transportation. Trips are subject to driver and vehicle availability, lawful routing, weather, traffic, vehicle capacity, and safety requirements.

Authority and compliance

TJ Legacy LLC Transportation will maintain for-hire passenger transportation authority, driver qualifications, vehicle maintenance, insurance, and operating practices required by applicable Wisconsin, Illinois, and federal law for the trip type performed. The Client is responsible for giving TJ Legacy accurate passenger, route, program, funding, pickup, drop-off, accessibility, supervision, and special-care requirements before service begins.

Wisconsin passenger carrier notice

Wisconsin intrastate passenger transportation for compensation may require motor carrier operating authority unless a statutory exception applies. This agreement does not convert TJ Legacy into a school bus operator, medical provider, ambulance provider, child care licensee, or government program administrator unless a separate written addendum is signed.

Passenger-Specific Services

Requirements for minors, senior passengers, accessibility needs, and funded programs.

Daycare, minors, and school-adjacent rides

For child, daycare, youth, or school-related transportation, the Client must provide parent or guardian authorization, emergency contacts, health-care provider contacts when required by the Client program, pickup and release rules, approved adult contacts, and required child restraint systems. The Client remains responsible for program licensing duties and child supervision rules that apply to the Client.

Senior, disability, and accessibility services

TJ Legacy does not provide medical care, emergency medical transportation, lifting, restraint, medication administration, or clinical monitoring. The Client must disclose wheelchair, mobility, attendant, door-through-door, cognitive assistance, service animal, and accessibility needs before dispatch so TJ Legacy can confirm whether the requested ride can be safely provided or whether another provider is required.

Federal and civil-rights terms

TJ Legacy will not discriminate based on race, color, national origin, disability, age, sex, religion, or other protected status. Where a Client uses federal, state, municipal, school, health-care, senior, or human-service funding, the Client must provide the specific contract clauses, Title VI, ADA, language-access, background-check, documentation, and audit requirements that apply to that funding before service is accepted.

Rates, Billing, and Changes

How the service bank, additional charges, invoicing, and cancellations work.

Rate model and disclosed additional charges

The minimum self-service service bank is $250.00. That minimum includes an estimated base mileage allowance but does not include conditional incident fees or government-imposed charges. Shorter plans carry a higher added-mile rate because dispatch, administration, vehicle readiness, insurance, and driver time are spread across fewer miles. Higher-mile plans receive lower additional-mile rates in volume tiers. The mileage and spend calculator is an estimate. The Additional and Incidental Fee Schedule shown in this agreement is incorporated into the contract and states the amount or calculation method for every listed charge.

Scheduling and use of service bank

Estimated miles are not guaranteed. The contract budget is a service bank applied against accepted trips. Unused balances, overages, renewal terms, invoices, and additional deposits are handled under the payment terms in this agreement unless TJ Legacy approves a different written arrangement.

Payment

Invoices are due according to the billing cycle selected by the Client and approved by TJ Legacy. TJ Legacy may require a deposit, prepayment, or account replenishment before dispatch. Past-due balances may pause service. Chargebacks, returned payments, collection costs, and attorney fees may be charged to the Client to the extent allowed by law.

Cancellation and no-show terms

Scheduled rides should be cancelled at least 24 hours in advance when possible. The disclosed no-show fee applies only after the stated grace period. Long waits and Client-requested or Client-caused trip changes are charged only under the disclosed fee schedule. TJ Legacy will not charge both wait time and special-event traffic time for the same minutes.

Fee authorization and invoicing

Conditional fees apply only when the stated event occurs. Pass-through tolls, parking, airport charges, taxes, and government surcharges are billed at the amount imposed by the third party or government. When reasonably known before dispatch, TJ Legacy will include them in the trip estimate; otherwise they will be itemized on the invoice. No undisclosed mandatory TJ Legacy fee will be added. Client-requested charges outside this schedule require written approval before they are incurred, except emergency expenses reasonably necessary to protect passengers, the vehicle, or the public.

Safety and Legal Terms

Passenger conduct, records, electronic acceptance, and governing law.

Passenger conduct and safety

Seat belts and child restraints must be used where required. No smoking, vaping, illegal drugs, open intoxicants, weapons, harassment, threats, or unsafe conduct are allowed. Drivers may refuse, pause, or end a ride when safety, legal compliance, passenger conduct, weather, traffic, road conditions, vehicle capacity, or dispatch limitations require it.

Records, privacy, and electronic signature

TJ Legacy may keep contract, trip, invoice, signature, IP address, timestamp, and operational records for business, insurance, compliance, and legal purposes. By signing electronically, the signer confirms authority to bind the business and consents to electronic records and signatures under applicable federal and Wisconsin law.

Relationship of parties and governing law

The parties are independent contractors. This agreement is governed by Wisconsin law unless controlling law requires otherwise. Any unenforceable term will be narrowed or severed without affecting the rest of the agreement.

Fee schedule

Additional and Incidental Fees

After reviewing the operating terms above, review the conditional charges that may apply to a trip. These fees are itemized on the estimate or invoice.

Special-event traffic $60/hour Extra vehicle and driver time caused by event closures or congestion, billed in 15-minute increments after the first 15 extra minutes. The same time is not also billed as wait time.
Route change $25 + mileage Client-requested destination or route change after dispatch, plus added miles at the contract mileage tier and any resulting pass-through costs.
After-hours request $35/trip Pickup scheduled from 10:00 p.m. through 5:59 a.m. local time. A request made less than 12 hours before pickup is $50 instead; the two fees are not combined.
Standard cleaning $150/incident Cleaning beyond ordinary vacuuming and wipe-down caused by the passenger or Client.
Biohazard or excessive cleaning $300 minimum Bodily fluids, smoke, prohibited substances, or comparable contamination; billed at documented actual cost when it exceeds the minimum.
Vehicle damage Documented actual cost Passenger- or Client-caused repair, replacement, towing, or professional remediation, plus documented loss of use up to $75/day. Normal wear is excluded.
